Transform 2026 Delegate Registration

Please submit this form by September 14, 2026. NOTE: This is not the same thing as registering for the conference. This is merely enrolling your delegates as delegates to vote at the Annual Business Meeting, September 24, all of whom must be registered. To register, go to http://efcamidwest.org/conferences.
